The Universal Assessment Instrument (UAI) is completed with the local Department of Social Services (DSS) Community-based Screening Team and requested through the local DSS by calling Intake at 540-586-7750 ext. 2276 during normal business hours.

Individuals who are Medicaid eligible or will be Medicaid eligible within 180 days of placement and who are seeking Medicaid long-term services and supports shall be screened within 30 days of the individual’s request for a screening to determine their need for the service.

Additionally, screenings are required for individuals who are applying for Programs for All-Inclusive Care for the Elderly (PACE) whether or not the individual is eligible for Medicaid.

Bedford County DSS completes UAI screenings for adults only. The Department of Medical Assistance Services has a contract with the Virginia Department of Health to administer child screenings in Virginia.
